التخدير الموضعي / local anesthesia 💉

⬜ طرق التخدير الموضعي / methods of local anesthesia 

1- التخدير الموضعي بالمس topical

2- التخدير الموضعي بالأرذاذ spray

3- التخدير الموضعي بالحقن injection

⏹الطريقة  الاولى : تكون على شكل مرهم او محلول يحتوي على تخدير موضعي نستخدمها قبل الحقن 

⏹الطريقة  الثانية : هو ماده xylocaine spray  اكثر شي نستخدمها للأطفال وايضا قبل الحقن 

⏹الطريقة الثالثة وهي الاهم  تقسم الى  قسمين 

⏩1- Infiltration 

⏩2- Nerve block 

نجي لـ  infiltration 

⏺ two kind could be differentiated :

1- soft tissue 

2- bony tissue 

 

هاي  الطريقة تكون  اسهل من البلوك واكثر شيوعا تستخدم بحالتين : 

⏹1- all maxillary teeth 

⏹2- anterior teeth of mandible 

🔃 هسه نجي للطريقة شلون ننطي لكل سن 

1- maxillary (central incisors + lateral incisors +canine +premolars):   buccal and palatal injection 

buccally  ننطيها ب vestibule 

palatally  ننطيها بنهايه الروت 

2- maxillary first and second molars : buccal injection in point betwwen two buccal roots and give palatal injection   وتكدر تنطي buccally  اثنين mesio and disto 

3- maxillary third molar : buccal injection between second and third to avoid injury pterygoid plexus of veins  ويصير تنطي للسن buccally بس للأمان ننطي بين second and third  اما palatally  ايضا ننطي بين second and third  حتى ليصير damage  ب lesser palatine nerve 

4-Mandibular (central + lateral + canine ) : labial and lingual injection 

⬛ الطريقة الثانية البلوك تتقسم الى 

◽1- Inferior  alveolar nerve block /IANB 

◽2- Mental block 

🔻نجي لـ  lANB 

هاي الطريقة تخدر لنا inferior alveolar nerve + lingual nerve +long buccal nerve 

تعتمد هالطريقه ع  sensation  تدخل نيدل من جانب الاخر بين 4و 5 وتدخلها ب medial aspect of ramus  تقريبا 15 cm  بعدين بس تغير الاتجاه ما اكدر اوضح اكثر حاولوا تشوفون مقاطع ب يوتيوب حتى تتاكد تخدر البشنت اسأله تخدرت الشفه والخد  وجزء من اللسان..  

⛔ complication if insertion of needle in high level 

1-trismus and sorness 

2- insertion of lateral pterygoid muscle and auriculotemporal nerve 

⛔ complication if insertion of needle in low level 

1- trismus and pain and deposited medial pterygoid muscle 

2- deposited parotid gland and posterior facial vein 

🔻 نجي لـ  mental block

هالطريقة تخدر لنا اسنان الفك السفلي =  central +lateral + canine + premolars

تخدر mental nerve + incisive nerve 

بالنسبه اليه هالطريقه عندي اقل استخدام اعتبرها بس نظري كطبيب اسنان لازم تعرفها،  اني استخدم البلوك دائما وكل واحد ورأيه..  

🔃 نجي  شلون ننطيها 

ندخل النيدل حوالي 2 mm  فوق mental foramen  حتى ميصير damage ب mental nerve  بين 4 and 5  وبزاوية 45  postero anterior  

 

هذا ملخص للتخدير الموضعي injection طبعا اكو طرق اخرى بس حبيت انطيكم الاسهل والاكثر استخداما 

⚠ contraindications of local anesthesia 

1-patients with hyperthyroidism 

1- patients with hepatitis 

3- patients with renal trouble 

4- presence of infection 

📛 ملاحظات مهمة جدا 

1- لاعطاء تخدير infiltration بدون الم

لا نقوم بدفع النيدل على ال vestibule لتجنب حدوث touch bone الذي يكون مؤلم عادة بل نقوم بسحب الخد (او الشفة) وبحركة سريعة على النيدل الموضوعة بجانب ال vestibule... الحركة السريعة ستشتت انتباه المريض ولن يشعر بدخول الابرة... فقط الم دفع سائل التخدير الذي ممكن نتجاوزه بال slow injection

2- ثني النيدل قليلا من الطرق التي تسهل ال inferior alveolar nerve block بسبب ال anatomy في هذه المنطقة حيث يكون مكان التخدير المناسب داخل concavity في العظم 

وكذلك تسهل ال palatal infiltration وال lower lingual infiltrationوخاصة في ال anterior region... لانها تعطي تحكم اكبر في المناطق الصعبة الوصول
